Uses and Disclosures: We may use or disclose your PHI for treatment, payment, and healthcare operations including administrative and quality assurance activities.

Other Permitted Uses: We may disclose PHI as required by law, for public health activities, or to prevent a serious threat to health or safety.

Your Rights: You have the right to access your records, request corrections, request restrictions, request confidential communications, and receive a copy of this Notice.

Complaints: If you believe your privacy rights have been violated, you may file a complaint with Synergy TMS or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
